Local Anesthetics Now More Widely Used in Cosmetic Surgery
A new technique being developed in the United Kingdom may help many patients feel safer and more secure during breast augmentation surgery. Surgeons at the Westbourne Clinic in Birmingham, UK, according to a recent Daily Mail article, are completing complicated cosmetic procedures using only a combination of sedatives and local anesthetics. Traditionally, such operations require a patient to be “put under,” or administered a general anesthesia which renders them completely unconscious.
General anesthesia has a few drawbacks. First off, about a third of patients that are put under using general anesthetics experience nausea and disorientation upon waking. While that works to deter some, the biggest scare factor is probably the horror stories that are regularly circulated about general anesthesia. Potential complications such as nerve damage, paralysis, remaining “awake” but being unable to speak, and even death cause a fear reaction in many patients that prevents them from considering operations that require being put under. The truth is that these complications occur less than once in 10,000 cases, but for some the fear still remains.
Nuala McEwan, a patient who recently underwent breast augmentation at the Westbourne Clinic, was put at ease by the fact that she wouldn’t be rendered completely unconscious.
“I had looked into having a breast enhancement but had been put off by the idea of a general anesthetic,” she said.
“When I heard that surgeons could perform the operation with a local anesthetic, not dissimilar to one used by a dentist, I knew it was what I had been looking for.”
The technique being fine tuned at the Westbourne Clinic involves administration of local anesthetic at the site of the operation, as well as the use of sedatives to help patients relax and remain motionless. FDA deems silicone breast implants safe
The Food and Drug Administration (FDA) recently released a statement confirming that new medical studies have demonstrated the general safety of silicone breast implants. The studies were performed by Johnson & Johnson’s Mentor Unit and Allergan Inc., two well known and long standing breast implant manufacturers.
Silicone breast implants were removed from the market in 1992 by the FDA on the grounds that the product’s manufacturers had not demonstrated the safety of the implants. From 1992 until 2006 saline implants dominated the market. Saline breast implants consist of a bio-compatible pouch filled with a non-toxic saline solution. While they look and feel somewhat like silicone implants and are well known for their safety and reliability, they are generally less sought after due to their watery, less than life-like texture.
During the fourteen year ban on silicone implants in the United States, researchers were unable to link the implants with any long term adverse health effects. Failure to identify any serious negative side effects associated with the implants led to their reintroduction into the market. The FDA asserted, however, that the reintroduction must be accompanied by new studies concerning the long term ramifications of receiving breast implants.
After the ban on silicone implants was lifted, Allergan and Mentor both began independent studies to confirm that their products adhered to the highest health and safety standards. Their studies monitored some forty thousand women who had undergone breast augmentation surgery, and documented the success of the procedures as well as the reliability of the implants at several year intervals after the initial surgery.
While maintaining active, accurate records of as many as forty thousand patients proved to be a complicated and difficult task for both Mentor and Allergan, the studies thus far have delivered some promising results. The FDA’s statement released late last month concluded that silicone breast implants are generally safe, and that only about one in five women experience complications within ten years of an implant procedure.

Plastic and orthopedic surgeons assist in Haitian earthquake relief
Organized by the British Association of Plastic Reconstructive and Aesthetic Surgeons in concert with Medical Emergency Response International, a team of surgical experts were deployed to Haiti following the earthquake which took place on January 12, 2010. The team was landed in country just three days after the catastrophe, and were performing limb-saving surgeries by the 20th of January.
A new report summarizing the successes of the operation, published in an industry magazine owned by the American Society of Plastic Surgeons, has just been released.
The team of surgeons followed a combined orthopedic and plastic surgery approach to minimize the need for amputations and to help victims of the Haitian earthquake retain both form and function in severely injured limbs. With a primary focus on avoiding amputation, the team of five plastic surgeons, five orthopedic surgeons and five anesthetists performed three hundred and forty-eight surgeries in total. The surgeries were performed on just under one hundred and fifty patients who had sustained crush injuries or similar trauma from falling rubble and other accidents.
While the orthopedic surgeons repaired breaks, fractures and splinters of the bones, the plastic surgeons worked to reconstruct skin and underlying tissue while recovering vascular systems and effecting aesthetically correct bodily repairs. As the brunt of the Haitian catastrophe was weathered, the influx of patients with bone damage declined markedly. By the end of the relief operation, it was discerned that about three quarters of the surgeries had been primarily soft tissue reconstruction — work performed by plastic surgeons.
Many plastic surgeons expend their professional energies providing aesthetic embellishments like breast augmentations, face-lifts and tummy-tucks. The experience in Haiti, however, illustrates the critical role that those same surgeons can play in the recovery of normal bodily appearance and functionality. The ten week trip helped to avoid a huge quantity of amputations and massively improved the quality of life for nearly one hundred and fifty victims of the disaster.
“This experience shows that a favorable amputation rate can be achieved,” write the authors of the new report, “and the changes in work load over time demonstrate the benefit that an ortho-plastic limb salvage team can provide in the early stage of disaster relief.”

BOTOX could become better regulated
BOTOX is among the top used anti-aging treatments in the world. Last year almost two and a half million people were injected with the fast acting substance in order to achieve a smoother, more youthful appearance.
It’s no surprise the treatment is so popular. Between its wide availability, its proven, near instant efficacy, and its incredibly affordable cost — around $400 per treatment — it makes sense that nearly one in every one hundred and thirty Americans has used BOTOX to put off the signs of aging for another year or two.
One concern about the wrinkle erasing substance, however, is that just about anyone can legally administer it even though doing so requires multiple injections delivered to sensitive, nerve-rich areas of the face. The potential for mistakes is such that botched BOTOX treatments have become a trendy mode of gossip around the country. For every couple of hundred treatments, someone ends up with less-than-desirable results.
While BOTOX itself is a controlled substance — meaning only a doctor can purchase it — anyone is permitted to administer it regardless of their qualifications. That may be about to change.
“There’s no set or approved curriculum or licensure or anything for that,” says Dr. Lori Stetler who practices out of Dallas. She went on to express concern about the potential mistakes that could result from untrained or poorly trained technicians delivering the treatments.
The Texas Medical Board is set to discuss the further regulation of BOTOX delivery this week. One possibility is that BOTOX delivery will be limited to doctors, nurses and doctor’s assistants — a big step towards safer administration of the youth restoring treatment. If new regulations are passed in Texas, a tighter control of the substance may catch on in larger spheres around the country.
“I like the idea,” says Dr. Stetler of the medical board’s considerations, “that they are looking into and hopefully will get rid of some of those people who are harming the public.”

Non-invasive cosmetic procedures on the rise
Non-surgical cosmetic procedures are out-pacing their surgical counterparts at an increasingly surprising rate. Chemical peels, microdermabrasion, laser treatments and other in-and-out procedures have thrown the doors of cosmetic surgery open to a much larger customer base than ever before.
Full blown plastic surgeries, on the other hand, are by no means experiencing a drastic decline. To the contrary, non-invasive procedures are simply picking up at an extraordinary rate.
